Our Community Support Services support individuals and families/whanau to gain access to their community and live ordinary lives. We:

  • provide access to information, skills, supports and resources.
  • link people with disabilities and their families/whanau with support in the community where     they live, and co-ordinate community options for individuals based on their personal goals.

Our community support includes:

  • referrals to other services offered by CCS Disability Action or other organisations
  • personal planning
  • co-ordinating flexible supports (e.g. in-home care) appropriate to each person’s needs
  • disability information, awareness and advocacy
  • liaison with the education, health and welfare sectors.

The service also supports people in addressing issues in their lives, if requested, such as building relationships with others, accessing social and recreational opportunities, or support with housing and employment.

We work to ensure that children, young people and adults with disabilities and their family/whanau have access to all services appropriate to their needs. We offer flexible solutions that enable each person with a disability and/or their family/whanau to access to specific skills and resources that enable them to achieve increased independence and enhanced social interaction.

Our community-based approach for children and adolescents is specifically designed to support and work alongside families/whanau. Our services provide families with the support they need to stay connected to wider family and community. The intensity, duration and frequency of supports therefore reflect individual need and change over time.
